Purpose and Structure
The European Glaucoma Society (EGS) released the 5th edition of its Terminology and Guidelines for Glaucoma to support ophthalmologists in managing individuals with, or at risk of, glaucoma. This edition emphasizes evidence-based recommendations and is divided into two parts:
- Part I: Addresses 16 key clinical questions with evidence-based recommendations, includes sections on patient concerns and communication, epidemiology, landmark studies, and cost-effectiveness considerations.
- Part II: Covers patient examination, classification and terminology, and treatment options.

Key Updates
- Evidence-Based Approach: Recommendations are based on systematic reviews and landmark studies, with evidence quality and recommendation strength graded accordingly.
- Clinical Practice Recommendations:
- Prostaglandin analogues are recommended as the first-line medical treatment for open-angle glaucoma.
- Central corneal thickness should not be used to adjust intraocular pressure measurements.
- Gonioscopy remains essential; imaging devices do not replace it for angle-closure diagnosis.
- Optical coherence tomography should not be the sole tool for early diagnosis or monitoring.
- Terminology Revisions: The classification "normal tension glaucoma" has been removed, and the use of cup-to-disc ratio is being phased out.
- New Pharmacological Options: Introduction of rho kinase inhibitors as a treatment option.

Patient-Centered Care
The guidelines highlight the importance of addressing patient concerns, including anxiety related to glaucoma, and ensuring clear communication to bridge information gaps.
